You only complete the queen/king quest, and therefore obtain the queen/king, if you do the following steps
IN THIS ORDER:
1. become a baron and get a townhall to level 9 (then level 8 cap quest shows up in quests)
2. cap a level 8 NPC (then level 10 cap quest shows up in quests)
3. cap a level 10 NPC (then queen/king of golden helm shows up in items)
If you do these out of order, you'll get the level 10, for example, but it won't get you the queen/king
So follow the steps precisely
